About Our Assessment
The Agency provides comprehensive, in-person assessments to determine the appropriate level of care for nursing home, personal care or in-home care provided through Community Health Choices or the LIFE program. The assessment is based solely on the functional abilities of the consumer.
These assessments are mandatory for individuals seeking Medical Assistance reimbursement for nursing home care or state directed in home care programs.
